*The church has 21 round tables, 7 rectangular tables, and approximately 165 folding chairs. Please indicate how many round tables, rectangular tables, and chairs you will need.

By signing this application, I understand that:
1. Smoking is not permitted inside any part of the church building.
2. Alcoholic beverages and illegal drugs are not permitted on any part of the church property.
3. I claim responsibility for the conduct of my group and guests while on Church Property.
4. Approval for use will not be granted without the Senior Pastor's approval.
5. If application is for a shower, party or reception, applicant is responsible for setup of tables and chairs (unless otherwise noted), and all food preparation and serving, if necessary. In addition, all other arrangements with caterers, cooks, organist, pianist, and audio/visual recording technicians are my sole responsibility. All such arrangements will be disclosed to the Board of Deacons or the Church Secretary in advance of the date of the event.

Deposit, Payment, Release & Indemnity Agreement, and Waivers
A deposit of $250 is required to hold the reservation. Unless other arrangements have been made, all remaining fees must be received at least one week prior to the event.

Once your application has been received and processed, you will receive an email with the amount owed, a copy of the Release and Indemnity agreement, and any additional required waivers.

Deposit, waivers, and Release and Indemnity agreement should be returned to 2225 Grandview Avenue, Monroeville, PA 15146. Please make checks payable to New Covenant Presbyterian Church. Contact the church office with any questions at 412-856-5717 or office@newcovpca.org.
