WKMB Membership Contract and Fee Payment
This online form combines the blue Marching Band Contract, the orange Fee Payment, and pink volunteer interest form for our registration process. Once these forms are complete, you will be directed to our Fee Payment store to finalize your enrollment in the 2020 Worthington Kilbourne Marching Band. We have extended registration to April 9. For those that would like to pay using the physical forms we distributed and check, please mail the physical registration forms to: James DeFrancesco, 8508 Bridletree Way, Columbus, OH 43235. Please note that payment in full -OR- initial $125 payment AND all uniform and miscellaneous fees are required to finalize registration. Parents are also welcome to register online and send checks for payment to the address above. All checks should be made payable to "WK Band Boosters".

By signing this contract, we are making a commitment to join this organization. We understand that by joining this organization, the student agrees to abide by all the rules of our band handbook (found on our website) and school. We understand that being a member of such an elite ensemble is a privilege and not a right. Furthermore, the student plans to attend every scheduled rehearsal and performance throughout the summer and fall as listed on the official calendar of the marching band. We understand that should a student decide to leave the WKMB, the Cancellation Policy found in our marching band handbook shall be adhered to strictly without exception. We understand that the student agrees to follow the established Co-Curricular Drug and Alcohol Policy and maintain academic eligibility requirements set forth by the Worthington City Schools administration. We understand that the student must provide a completed physical card to be eligible to participate. We also understand that we will be responsible for payment of any lost or damaged uniforms/parts for the uniform assigned to the student. We understand that failure to observe these terms may result in the student not being able to participate in some or all of our Marching Band events, performances or rehearsals.
