Sacred and Profane Survey

The purpose of this study is to examine the degree to which individuals define various events and social phenomena on a scale of religious symbolism. Further, the study will attempt to discover whether there is a relation between religious affiliation, religious observance , and other demographic information including age, gender, educational level, etc. and the way in which individuals define social phenomena.

    Shawnee State University CONSENT FORM FOR PARTICIPATION IN RESEARCH STUDY 1. Study Title: “Reexamining Religion” 2. Performance Site: Shawnee State University 3. Purpose of the Study: The purpose of this study is to examine the degree to which individuals define various events and social phenomena on a scale of religious symbolism. Further, the study will attempt to discover whether there is a relation between religious affiliation, religious observance and the way in which individuals define social phenomena. 5. Subject Included: Individuals enrolled at Shawnee State University 6. Number of Subjects: 300 7. Study Procedures: Individuals will be asked to complete an online survey rating each social phenomena on a sliding scale. 8. Benefits: Subjects will receive extra participation credit in their SOCI 1101 class. Additionally, the study may yield valuable information about how individuals define social phenomena based on religious affiliation and religious observation as well as other criteria. 9. Risks: The only study risk is the inadvertent release of sensitive information collected during the study. However, this study is anonymous and the only information that would identify the student, the student ID number, will be kept separate from the findings of the study and destroyed once it has been used to award extra credit for the course. 10. Right to Refuse: Subjects may choose not to participate or to withdraw from the study at any time without penalty or loss of any benefit to which they might otherwise be entitled. 11. Privacy: Results of the study may be published, but no names or identifying information will be included in the publication. Subject identity will remain confidential unless disclosure is required by law. All documents will be stored in a locked storage closet in the Social Sciences Department for three years, at which point the documents will be destroyed. 12. Signatures: I verify that I am 18 years of age or older. The study has been discussed with me and all my questions have been answered. I may direct additional questions regarding study specifics to the investigators listed above.
