NHCC Sports Camp Registration

Join us for a fun week of sports!  TORCH SPORTS MINISTRY and NEW HOPE COMMUNITY CHURCH are partnering together to run a sports camp including: Basketball, Baseball/Softball, Field Hockey, Flag Football, Soccer (just to name a few).  Campers will have the opportunity to play various sports while learning good sportsmanship based on Biblical principles.  TORCH SPORTS MINISTRY AND NHCC will have experienced coaches working with your children.

Dates:  June 27  - July 1, 2022

Time:  9 am - Noon

Cost:  $85/player*
Please bring a check on the first day of camp made payable to TORCH SPORTS MINISTRY.

*All proceeds will benefit the TORCH SPORTS MINISTRY after all incurred expenses are paid.

• RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT CONCERNING COMMUNICABLE DISEASES In consideration of being allowed to participate, or providing express permission for my child to participate (collectively "Player") in any way in any program, event, or camp activity sponsored by the New Hope Community Church (collectively “Activity”) and the Torch Sports Ministry, I the undersigned, acknowledge, appreciate, give full consent, and give express permission, that:    I am aware there are risks, including exposure to directly or indirectly arising out of, contributed to, by, or resulting from an outbreak of any and all communicable disease, including, but not limited to, the virus “severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)”, which is responsible for Coronavirus Disease (COVID-19) and/or any mutation or variation thereof. *
I understand that the Player’s participation is completely voluntary.  In consideration of the Player being permitted to participate in the Activity, I, for myself, and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS THE NEW HOPE COMMUNITY CHURCH & TORCH SPORTS MINISTRY and their collective officers, officials, employees, agents, volunteers and representatives (collectively “RELEASES”), from any and all claims, demands, losses, and liability, including reasonable attorney’s fees, arising out of or related to any ILLNESS, INJURY, DISABILITY, OR DEATH that Player may suffer, or the Player’s family member may suffer from exposure to Player, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASES OR OTHERWISE, to the fullest extent permitted by law.   *
Further, I agree to cooperate and comply, or I agree to direct the Player to cooperate and comply, with all rules, directions, and instructions of the coaches or other supervisory personnel in charge of the Activity (collectively “Supervisory Personnel”), especially as it relates to following of NEW HOPE COMMUNITY CHURCH and TORCH SPORTS MINISTRY COVID-19 guidelines (collectively “Guidelines”). I understand that Supervisory Personnel have the authority to remove the Player from the Activity and deny the Player’s participation in the Activity, including during times when the Player fails to comply with such Guidelines.   *
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. *
As a parent/guardian I also acknowledge that my child (children) will be screened for symptoms of Covid-19 at home by a parent/guardian before I leave for camp and I will not send my child (children) to camp if they have shown any of the following symptoms within 24 hours.  ● Nausea of vomiting  ● Fever greater than 100.4  ● Runny nose  ● Sore throat  ● Shortness of breath  ● Cough  ● Diarrhea  ● Weakness of malaise  ● Headache  ● New loss of taste or smell *
