Volunteer Background Check

NOTICE TO VOLUNTEERS/VISITORS REGARDING BACKGROUND INVESTIGATIONS
Due to the importance of maintaining a safe environment for students and staff, all volunteers and/or visitors who may have contact with students/staff are required to have a current limited criminal history background check on file with Zionsville Community Schools.

A background screening report may include information concerning any police, criminal, or motor vehicle records will be obtained in connection with your application as a volunteer with the Zionsville Community Schools.

A copy of the report will be made available upon request should information contained in the limited background check prohibit volunteerism in Zionsville Community Schools. A background check must be conducted every five years for volunteers. A volunteer/visitor shall be required to report arrests or the filing of criminal charges to the volunteer coordinator within two (2) business days of the occurrence. A thorough review of each reported conviction will determine whether or not the volunteer/visitor can continue to work within the Zionsville Community Schools.

AUTHORIZATION By signing below, I hereby voluntarily authorize Zionsville Community Schools to obtain an Indiana State Police Limited Criminal History Report (IC 10-13-3-27).
