2025-2026 Registration Form Season #14
Welcome to Season #14! We are excited to welcome you back or if you are new, we are excited to have as part of the EDA Family!  Each season a new registration form is required along with an updated credit card authorization form.  We understand that this can be a pain, but we need to ensure we have the most current information on your dancer on file to ensure we can communicate with you throughout the season.  All families are required to be setup on automatic payment; families can choose either the first or the fifth of each month for payment to be processed  

Please fill out the following fields to register your student(s) with Elite Dance Academy and once your form has been received, we will complete registration using information provided including, charging the card provided for the total amount due to complete registration which include annual Standard membership fee or Premiere membership fee and first full month of tuition. Registration will not be complete without payment being processed, we will not be able to enroll your dancer into their class(es).  

Pay for the whole year in advance and save 10% off monthly tuition and pay no annual standard membership fee this does not include VIP membership fees.  If your card provided is declined when registering, we will contact you and you will have 24 hours to make payment, or a new registration form will be required.

Tuition and Membership Fee is due at the time of Registration.  Card listed on form is card that will be placed on autopayment for entire season. There are no refunds or credits after your dancer's registration form has been received.

Elite Dance Academy Liability Waiver and Acknowledgement of Risk:
I understand and agree that in participating in any dance class, workshop, rehearsal, or performance, there is a possibility of physical injury or death. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child during any of Elite Dance Academy classes, rehearsals, performances, or activities. I also exempt, release, and indemnify Elite Dance Academy, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes for action whatsoever out of or in connection with participation in any classes or activities conducted by Elite Dance Academy. I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold Elite Dance Academy, its owners, agents, volunteers, assistants, employees, guest artists, faculty members and/or students liable for such damage, loss, injury, or death. I understand that I should be aware of my physical limitations and agree not to exceed them. If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights.

Tuition: Tuition prices are based on (12) twelve-month calendar. Tuition is broken up into 12 equal installments. Therefore, tuition remains the same each month regardless of the seasonal breaks and if any month has less or more than four weeks. No refund or credits

Payment of Fees:  All families will be required to enroll in autopayment for dance tuition. This is aimed at enhancing convenience and efficiency in managing tuition fees.  You will have the option to choose between two payment dates: the 1st or the 5th of each month. This flexibility allows you to select the date that aligns best with your financial schedule. Payments will be processed on date selected and any payments declining on that date will incur a $10 processing fee.  We kindly remind you that it's your responsibility to ensure that your card information is accurate and up to date, and your card is unlocked and that sufficient funds are available on the selected payment date chosen at registration. Students with unpaid accounts after the 7th of the month will not be allowed to attend classes. Late Fee: A late fee of $25.00 will be charged per student on unpaid tuition after the 7th of each month.  Tuition cannot be credited to another student's account and is nonrefundable. 

Forms of Payment Accepted: EDA accepts cash and credit card payments only.  No hundred-dollar bills please.  No checks!   Families can pay with cash or card inside the studio on or before the date they have chosen for automatic payment and their card on file will not be processed.

Withdrawals and Refunds: A written 30-day notice is required to withdrawal student from a class or classes. Withdrawal must be requested in writing at the studio office, not with teacher and will NOT be accepted over the phone. Withdrawal procedures are as follows: Completing and signing a withdrawal form provided by the studio office or submit form through our website. Families are responsible for tuition covering the 30-day notice.  No refunds/credits are given No Exceptions!

Dress Code: Required dance wear and dance shoes must be worn to all classes. See specific dress code for each of student’s classes.  Students are NOT permitted to wear jeans or shoes worn on the street or outdoors are not allowed in dance studios where class will take place. For safety reasons dancers are not allowed to wear jewelry.   Hair should always be pulled back in a bun for all classes. Any dancer that is continuously out of dress code may be asked to sit out of class, parents will be contacted, continued failure to follow appropriate dress code could result in termination of lessons. Important Attendance & Make-Up Policy Reminder

Make-up classes are allowed during July, August, and September to accommodate families with planned summer vacations. Any absences during this period must be made up within 30 days.

Starting October 1st, no make-up classes are allowed—NO EXCEPTIONS.

Please remember: We do not offer refunds, reductions, or pro-rating for missed classes. Each class is carefully designed for the specific group of students, and make-ups can disrupt the learning environment. Similar to sports, missed practices (classes) cannot be made up.

If class has already started, please stop by the front desk before allowing your dancer to enter the classroom.

If a student arrives 10 minutes or more late, they will not be allowed to participate in class that day.

Holiday Closure:  Make-up classes will be allowed for the Labor Day closure only.  A makeup class should be made up in a similar class style, level and age. These classes must be made up within 30-days. See the front desk for make-up classes. No make up classes can be scheduled in a full class.  No credits or refunds will be given.

Food and Drinks: Absolutely NO GUM, FOOD or DRINKS allowed in the DANCE ROOMS Except for a sealed leak proof water bottle of any type. Gum is absolutely not allowed on premises. Food and drinks must be consumed outside of dance room.  Water is allowed throughout the studio and dance rooms if it is in sealed container, (ex. water bottle, thermos etc.). Please put trash and recycle in appropriate areas. Keeping the studio clean is everyone’s responsibility. Students that violate these rules may be dismissed from the studio.

Substitution: Elite Dance Academy reserves the right to change or provide a substitute teacher if the regularly scheduled teacher is ill or otherwise unable to teach class. If a teacher is ill and the school cannot arrange a substitute, the missed class will be made up at a later scheduled date.

Social Media: I understand that I nor my child should ever post any negative comments regarding EDA parents, students, or faculty as well as negative comments regarding other studios, teachers and their students. Anyone found in violation of this policy will be removed immediately. Parents please monitor your student’s social media.

Pre-Approved Classes: Classes noted as pre-approved will need instructor’s permission prior to registering for class. These classes may require students to have certain number of years of prior dance training and skills. All new pre-pointe/pointe students will be required to provide a doctors’ release prior to enrolling in pointe class stating dancer has no limitations.

Injuries: Parents, legal guardians of minor students and adult students waive the right to any legal action for any injury sustained on studio property resulting from normal dance activity or any other activity conducted by student before, during or after class time

Parent’s responsibility to be aware of dates and events: It is the responsibility of the parent/guardian to be aware of all studio activities, such as Show and Shine weeks, recital rehearsals, recital picture day, recital show day and times, extra classes and dates studio is opened and closed. The studio will post all notices on the bulletin boards, as well as email, Band Ap and social media sites. It is the responsibility of the parent/guardian to inform the studio of any telephone and email changes.

Care of Student’s: The studio is not responsible for providing childcare before or after class. Students should not be left at the studio for excessive time before or after class. Any student not picked up within 10 minutes after class dismissal will be charged $15 for every 5 minutes they are not picked up. In the event of an emergency please contact the studio and advise them of the circumstances. Students under the age of 12 years old will not be allowed to leave the building without being accompanied by an adult. Parents’ will need to come into studio to pick up their student. This is for the safety of the student.

Students left past the 10-minute grace period will automatically be billed and card on file will be charged.

Drop off, Pick-up, Lobby: To maintain a calm and safe environment for our dancers, only one parent or guardian is allowed in the studio per child. Lobby access is reserved for families of dancers enrolled in Parent & Me (18–36 months), Twinkle Tots 2–3 yrs., Twinkle Tots 3–4 yrs., and Twinkle Tots 4–5 yrs. Dancers ages 5 and up must be dropped off and picked up at the front door. Dancers under age 12 may not cross the parking lot without an adult. New students experiencing separation anxiety may request lobby access for their first two weeks. To reduce distractions, we ask that all visitors keep voices low and ensure any accompanying children are seated, quiet, and supervised at all times. NO Running, yelling, or disruptive behavior will be permitted, and those who do not follow these guidelines may be asked to wait outside. We understand it can be hard to step away, which is why we’re excited to introduce Spot TV, a new feature beginning Season #14 that allows parents to securely view their dancer’s class from their phone.  Initials Below

Recital:  Registration for our annual holiday show will open in August and June Showcase registration will open in January. Dates are subject to change without notice.   Each family is required to OPT IN or OPT OUT of each recital.  Students enrolling after August 31st and March 1st may not be eligible for recital as we cannot guarantee the arrival of costume after those dates.  Dancers participating in recitals must attend class(es) on a regular basis. Additional shipping fees may apply to late orders.  Costumes will only be released if an account is paid including month of recital.  NO EXCEPTIONS!
Photo and Video Release: Elite Dance Academy is hereby granted permission to take photographs and videos of students to use in brochures, websites, posters, Facebook, advertisements, and other promotional materials the studio creates. Permission is hereby granted for the studio to copyright such photographs in its name. *

Authorization Needed: Automatic payment. All students registered at EDA will require to have a credit card authorization on file for monthly automatic payment.  Families will be able to choose the first or the fifth of each month.  If card declines on the day chosen for auto payment to be processed a $15 fee will be charged to the account. It is the family's responsibility to ensure proper funds are available and card is unlocked.  I authorize the Elite Dance Academy to charge the credit card indicated in this authorization form according to the terms outlined above.
