2018-2019 POB Scholarship Redemption Form

CONGRATULATIONS!
You have been awarded a Scholarship by the Perrysburg Orchestra Boosters to be used for private instruction for the remainder of the 2019 school year!

REDEMPTION DEADLINE:
- Form submission is required by Tuesday, April 30, 2019, or your scholarship could be forfeited.
- To receive payment, please complete the requested information below.
- Please let us know ASAP if unable to use the funds so they may be reallocated to an alternate student.

PAYMENT NOTES:
- The check will be mailed directly to your home using Huntington Bill Payment service. It will list the private instructor's name first, then "Attn: (Student name)", then student's address. It will be a small postcard sized envelope with three pull-off tabs to open it. Please take care not to throw it away or lose it. Checks must be deposited within 90 days or risk forfeiture of scholarship funds.

- Check turnaround time is usually within 5-10 business days upon submission of this redemption form. If our volunteer time allows, you may receive a confirmation note to the email you provide below once payment has been scheduled.

INSTRUCTOR SELECTION:
If a private teacher recommendation is needed, please contact your orchestra director. A listing of summer music camps is available here or upon request from your director: http://www.perrysburgschools.net/Links1.aspx.

If you opt to use the scholarship toward an alternate summer music camp, please indicate the camp. We will contact you for further necessary information. Most camps will allow split payment. We would indicate your student's name on the check. Payment will likely be sent directly to the summer camp on your camper's behalf, but redemption instructions may be slightly different. Proof of camp registration may be necessary. We will verify instructions once your redemption form has been submitted.

If selecting SOF! Summer Lessons for redemption, you must also complete the SOF! Summer Lesson Registration/interest and preferred summer time availability form distributed by Mr. Smith. Look for the email announcement as early as mid-March.

Some families have previously used the balance to continue extra lessons with their SOF! student instructor.
