Alberta Health requires all attendees, visitors, and staff of schools, daycares, preschools to complete a daily checklist prior to participating in the activity or program in order to reduce the risk of transmission of COVID-19.
Parents/Guardians are asked to check their child's temperature each day prior to attending the playschool. We ask that parents or guardians complete this form PRIOR to your child attending Caminitos Playschool.
This form must be competed EACH DAY of attendance.
Data collected is saved for 2 weeks (as per Alberta Health requirements). After 2 weeks, the information is deleted.
If you have more than one child attending the program, please complete a separate checklist for each child.
If you are visiting or volunteering at the Playschool (ex: volunteering in the classroom), please complete the Daily Checklist for Adults (https://forms.gle/GXu6ZSt6L6UaggUh8).

If your child has traveled outside Canada in the last 14 days, follow the Government of Canada Travel, Testing,
Quarantine and Borders instructions (https://travel.gc.ca/travel-covid), including any requirements for exempt travelers related to attending high-risk environments.
