• Our school district is conducting an evaluation of selected programs and student services. Parent input is an important part of this evaluation. Your participation in this survey will be used as part of our district and campus evaluation.
• For each descriptor listed in the survey, please assign a rating between 1 and 4 to indicate your opinion. A rating of “1” indicates Exemplary and a rating of “4” indicates unacceptable.
