By completing this form, you acknowledge that you are granting permission for Old St. Mary's faculty-staff to release your student(s) to walk home from Old St. Mary's after school until 4:45 PM.

This permission also applies to after school activities at the school or gym.

You are required to add the names of all students that you release to walk home -any students that are not identified in this form will not be released.
