EVCC is an EFC-EV community outreach ministry and a Christ centered educational organization serving our communities.  EVCC's mission is to equip students in a biblically based environment with music skills and knowledge, helping them to be become mature adults with sound body, mind and a positive attitude towards life.

EVCC Media Consent:

I hereby consent to the collection and use of my personal images by photography, sound or video recording.  I acknowledge these may be used on the EVCC website, social media, newsletter, yearbook or other publications including flyers.  I understand that no personal information will be used unless express consent is given.  (Except for yearbook, in which the student's names will be printed with their photo).  I also understand that my consent can be withdrawn at any time in writing to EVCC at 20625 La Puente Road Walnut, CA 91789 *

Waiver of Liability:

In Consideration of the possible risks involved, I hereby release EFCEV and its agents from any and all claims, whether legal or financial, that may arise or be caused by transportation to or from EFCEV, use of EFCEV and destination facilities or properties, ordinary negligence of the provider, or involvement with this organization.  This release is signed on behalf of the singer, spouse, heirs, administrator and assigns as a covenant to not sue in the event of injury, loss or death.

Authorization of Necessary Medical Care:

I give EFCEV and its agents, permission to authorize for the minors or for myself listed above any necessary medical care rendered by a physician licensed under the medical Practice Act.  I authorize the facility which has provided treatment to surrender physical custody of these minors to EFCEV upon completion of treatment.

Assumption of Risk:

Some of the risks involved with this organization and transportation to and from planned events are not limited to:  minor injuries such as scrapes or bruises; serious injuries such as broken bones or concussion; or even catastrophic events such as paralysis, loss of limb, or death.  By signing this legal document, I understand and appreciate the potential risks involved and I agree to assume responsibility of these risks for the minor or for myself.

EVCC Choir Regulations:

1. Students should arrive to classroom on time.  Choir rehearsal will be every Sunday from 3:15 - 5:30 pm.  Parents please notify teacher or secretary in advance if the student will be absent or leaving early.

2. Please
* Sign in and sign out your children
* No leaving the classroom without permission
* Keep compliance with EVCC's COVID Policy
* No talking without permission during class
* No running during recess
* Keep cellphones on silent during class
* No food or drinks in classroom except capped water bottle
* Keep playground and classroom clean
* Keep music folder, sheets, shirt and uniforms clean and in good shape

3. For the safety of your children, please drop off your children no earlier than 15 minutes before class start and pick up no later than 15 minutes after class ends.  EVCC is not liable beyond this time frame.  Please call the director or on duty staff member for emergency.

EVCC reserves the rights to charge a $30 late pick up fee for infringement of this policy.  If we do not hear from you 30 minutes after class ends, your children will be moved to the director's house.

4. Students are expected to join all choir activities and events.  If you have 3 or more absences prior to concert or if you are absent for the dressed rehearsal, you will not be allowed to join the concert.

5. Solo parts are chosen by the teacher based on the student's attitude, musical "sportsmanship" and musical talents.

6. Chamber Choir is EVCC's most outstanding choir.  It is by invitation and audition only, based on the student's personal musical talents and their ability to collaborate with other choir members.

7. The following will be awarded at the year end ceremony:
* Perfect attendance: no absences or late / leave early more than 3 times
* Most improved
* MVP
* Scholarship - given to graduating seniors based on their tenure *

Refund Policy
Partial or full refund can be requested before 9/1/2026.  We will deduct $20 for each week that has passed.  We will not refund any absent classes.  

There are no refunds after 8/31/2026.
