TMSA accepts only the following as excusable reasons for absence from school. Parents or guardians must notify the school (with this form) in the morning on any day their child is not attending. The excuse shall be submitted to the school office and filed as part of the student's school record. Absences which are not verified by parent contact with the office within 3 days will be viewed as unexcused. A student’s absence from school is excused by parent notification for a maximum of 10 days. Once a student reaches 10 absences (excused or unexcused), doctor verification is required for additional absences to be considered excused.

Completing this form will eliminate the need for you to call or email on the day of the absence. The form does not automatically correct the attendance. It is manually inputted, therefore please allow up to 5 days for the update.
