This form is to be used when reserving a district facility areas. Please allow 48 hours for this to be approved and posted on the calendar.

Weekend or non school events will be charged $20.00 an hour for custodial services as well as a $20.00 an hour light fee for each event. Custodial/light fees will be assessed prior to event. You will be notified with your calendar notification of the fees due. *

You are responsible for any damages incurred by your players, or family members in the facility during the time you have it reserved. *

Should damages occur you could become limited or removed from the facility . *

If there is a disagreement between two teams both could face removal or limited use of the facility. Make sure you have your facility approval notice with you. *

The building principals have the right to have you permanently removed from their building. *

If the building is closed the facility is closed. (no school for a teacher work day, inclement weather, quarantined for cleaning for example) Your event will be cancelled for that day.

Board Approved Facility Usage Fees
We will do our best to keep that date and time for you, but please understand that school related events take priority, if there is a conflict you will be notified ASAP to reschedule. We the undersigned hereby makes application to Camdenton R-III School District for the use of school premises and certifies that the information given above is correct. The applicant will exercise the utmost care in use of the school premises and property and will make good on any damage arising from the applicant’s use of said premise and property. We hereby agree to defend, indemnify and hold harmless the Camdenton R-III School District, Board Members, administrators, teachers, employees, volunteers, and agents from and against any and all liabilities, claims, damages, penalties, actions, suits, losses, costs, and expenses arising out of or in connection with the use of said premises. This agreement to defend, indemnify, and hold harmless specifically includes all claims arising out of any accident or other occurrence on or about said premises causing injury to any person or property. The undersigned has read and is in agreement with the rules and regulations for building. *
