I am voluntarily participating in yoga classes offered at Center School District and hereby accept any and all risks of personal injury, illness or other risks that may be related to the yoga classes. In consideration of my participation in the  classes, I hereby release Center School District, Cbiz, its subsidiaries and affiliated companies, Kyle Stern, and each of the party’s agents or representatives from and voluntarily waive any and all present and future claims, including negligence for property damage or personal injury arising from my participation in the yoga classes.I have read this form and fully understand that by typing my name in this form I am waiving any rights or claims I might have for damages in connection with these activities.
