AUSD School Change Request Form (2018-19)
Complete this form by March 30, 2018 if you wish to request that your child/children be transferred to a different school for the 2018-19 school year. You must submit one form for each child you are requesting to transfer. Requests received after the deadline will be added to the waiting list, but will have a lower priority than requests received on time.

Please note the following timeline:

• by June 30th: In the event that space becomes available that meets your request, we will conduct a lottery for students requesting that space.

• In June & July: If your child is selected in the lottery, we will notify you by email. At that time you will have 3 calendar days to respond to our offer, after which the space may be offered to the next family on the waiting list that is created in the lottery.

• In August: If space becomes available for your child, we will notify you by email. At that time you will have 1 calendar day to respond to our offer, after which the space may be offered to the next family on the waiting list that is created in the lottery.

• September 30th: After this date students will not be transferred until the following school year.
