Tahoma ID Badge Replacement
To request an ID badge replacement, please submit your request on this form. Replacement badges are issued for lost or broken ID badges. Please return any old ID badges to the Human Resources Department. Your Outlook email picture is what will be used. If you do not have an Outlook email photo on file you will be contacted to have your picture taken. You should receive your replacement ID badge within a week of submitting this form.
If you have any questions, please contact Cheryl Matava at 425-413-3422 or cmatava@tahomasd.us. Also, if you lose your badge, please contact Cheryl immediately so she can deactivate the access portion of your badge.
