Course Age Requirement: *
Age requirement for the course? Note: Classes are for ADULTS, 18 YEARS AND ABOVE. If you want to offer a class for those under the age of 18 years, New York State requires instructors to be fingerprinted AND CPR certified. Fingerprinting for educational institutions has been outsourced to IdentoGo (http://www.identogo.com/locations/new-york). The instructor is responsible for the fingerprinting process and for obtaining CPR certification, including cost. Once fingerprinted, instructors must meet with a N-W District notary to sign an affidavit to access prints.
Instructor Contract Agreement *
I agree to provide instructional services for the class above, as part of the Spring Session, to the Niagara Wheatfield CSD Community Education Program. I further understand that I will be compensated for my services according to the "Proposed Registration Cost Per Student" as stated above. *School is not in session September 27, October 14, November 11, 27, 28, 29, 2024.
