LYDIA LUNCH Collaborator Information Form
In order to correctly register song titles for Lydia Lunch, I need you to complete to following information for me as COMPLETE AND CORRECT as possible.  This will insure that you receive your performance royalties correctly without any difficulties.  KINDLY GIVE THIS YOUR FULL ATTENTION, AS THIS IS THE INFO THAT WILL BE REGISTERED WITH YOUR PRS AND WITH LYDIA LUNCH'S PUBLISHER.   Making changes after they have been registered is a timely and difficult affair -- so get it right to begin with please.

SONG TITLE(S) with time durations: *
If you’ve  co-written all the songs on the album, you need to list every title correctly spelled out (as on the CD); or write in all the titles if the info below is the same.  If you have several songs on an album with Lydia, and these have different publishers or differ in other info, you need to submit one form for each song title.  PLEASE NOTE: If there are numerous composers/writers, you need to provide information on each person as asked below (their PRS, Publisher, CAE numbes, contact info, etc.)   PLEASE ALSO INCLUDE THE TIME DURATION OF THE SONG (Minutes: Seconds).    So, for example write  "SONG TITLE" (03:33)
INFO ON HOW TO DIVIDE ROYALTY SPILTS
We usually split the royalty 50-50%.  That is, 50% to the text writer, and 50% to the composer, unless otherwise specified.  If you have a different understanding from this, now is the time to tell me. The same applies for the PUBLISHING interests.  If Lydia has also composed music with you, then she will be further entitled to another 25%.   So, for example this would be:

LYRIC WRITER:
Lydia Lunch, ASCAP (#1596819): 50%;    Publishing 50%
COMPOSER:
Other Person, BMI (#1234567): 50%;   Publishing 50%

- or -

LYRIC WRITER:
Lydia Lunch, ASCAP (#1596819): 25%,   Publishing 25%
Other Lyricist Name, BMI (#1234567):  25%, Publishing 25%
COMPOSER:
Other Person, ASCAP (#1234567): 50%, Publishing 50%

or similar.   Note that for the WRITER & COMPOSER, the full amount must total 100%.   Also for the PUBLISHER, the full amount accorded for each publisher must total 100%.  Still not clear?  Then please ask me!  


Who is/are the LYRIC WRITERS?  Full names please. Kindly write their PRS affiliation (that is, either ASCAP, BMI or other Performance Rights Society) followed by CAE number in parenthesis, then the percentage share of the WRITER share.  The CAE number is listed on any ASCAP, BMI or other PRS statement sheets, or you'll find it online in your account with these.    If no CAE then I’ll need your Social Security number.  If you are not associated with a PRS, please write NONE.  Note that if you are not associated with a PRS or publisher, you will not receive any income from performances.  ALSO, write the agreed income split shares in percentage.  So, this should look like, for example,  "Lydia Lunch, ASCAP (#1596819): 50%".   THE TOTAL SHARE OF WRITER'S SHARES SHOULD TOTAL TO 50%.   *

If you use a pseudonym, kindly state which artist name you want used.  Note that it should be the same name that is registered with your Performance Rights Society, such as ASCAP, BMI, or PRS, etc. (Usually your birth name)  Please do the same for each writer other than Lydia Lunch.

• On your track listing, you should always have the timing of the songs on the back of the CD.  
• You should remember to have the copyright info on the CD as well as in the booklet, preferably on the back.

• Barcode at bottom with white background.

• Catalogue number should appear on the product spine, and on the back.

• IRSC number info, such as
“IRSC:  NO-CER-0602010 to NO-CER-0602120”
This reflects a CD with 12 tracks and is a personalized number designated to your company from your PRS.  It has to do with mechanical collection and should be included in your mastering. Contact me if you need more info on this.
