Bookfulness Box Submission Form 

Bookfulness Boxes can be picked up on the first Monday of each month and books are due after 3 weeks. The goodies inside are for you to keep, but remember to return the box and the book! After submitting this form, you will get a confirmation email with the pick up date for your Bookfulness Box! 

Bookfulness Boxes are for only for patrons with a Rolling Meadows Library Card in 6th, 7th, or 8th grade.
