Payment is required to secure your spot. Teams will be accommodated on a first-come, first-served basis, with a maximum of 4 teams per division and a total of 3 divisions available. Due to limited space, payment is crucial to confirm your participation. For inquiries, text 734.658.5305.

All round-robin and semi-final games will be played as 3v3 half-ice matches. Championship games will feature 4v4 full-ice gameplay without icing or offsides. This event is designed for enjoyment, and teams are required to have both light and dark-colored jerseys; no numbers are necessary, and there will be no score sheets.

Parents are welcome to coach, making it a fun-filled night. If desired, teams can give their coach the night off. Each game will have one referee, and teams will track scores using a scoreboard on the bench. In the event of a tie, a head-to-head shootout, survivor style, will determine the winner.

Waiver: To participate in any activity, a signed waiver is mandatory. By engaging in the hockey program and associated events, I acknowledge and consent to the following:

I comprehend and recognize the risks inherent in the program, including the potential for injury, paralysis, and death. I willingly accept all such risks and assume full responsibility for my participation.

I commit to adhering to the rules, terms, and conditions of participation.

On behalf of myself, my heirs, assigns, personal representatives, and next of kin, I hereby release, indemnify, and absolve HoffHockey LLC, its officers, officials, agents, employees, instructors, coaches, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of the premises used for the activity (collectively referred to as "Releasees"), from any and all injury, disability, death, loss, or damage to person or property associated with my presence or participation, to the maximum extent permitted by law.

I release HoffHockey LLC from all liabilities related to both on-ice and off-ice activities.

I grant my full approval for my player to partake in the entire program, encompassing body contact and checking. This approval extends to all coaching staff, whether volunteers or contracted.

PARENTS/GUARDIANS OF PARTICIPANTS UNDER THE AGE OF 18:

As the parent/guardian with legal responsibility for the mentioned participant, I certify that I consent and agree to their release as outlined above. On behalf of myself, my child, and our heirs, assigns, and next of kin, I release, indemnify, and absolve the Releasees from any and all liabilities related to my minor child's involvement or participation in the physical aspects of the game of Hockey, including hitting, fighting, checking, and any and all body contact. Additionally, I grant full approval for my player to participate in the complete program, including body checking, hitting, and any drills or gameplay involving contact if they are below the current age requirement for body contact, to the fullest extent permitted by law.

I have thoroughly read this release of liability and assumption of risk agreement, fully comprehend its terms, and sign it freely and voluntarily without any inducement on behalf of myself and/or my minor child.
