Please complete the following form to apply for clinical services.

All sessions are scheduled on Tues/Thurs 9:00-5:00. The type of service offered (in person vs. telepractice) varies per speciality area. When applying, be sure to factor in whether you will be able to commute to the clinic or have the technology to participate in telepractice services. If you have any questions about this, please reach out and we can help you.
