Intake Form - Montana West 2024
The 2024 Montana West NSDA Qualifying  Tournament is committed to providing a safer and more equitable speech and debate competition. In order to do so, we ask that all attendees be aware of and follow our harassment and discrimination policy.  

We have an NSDA Certified Belonging and Inclusion Advocate (BIA) available throughout the tournament. The BIA for the 2024 Montana West District Tournament is Coach Jennifer Hermanson, Helena High & District Board Member.

Any person who believes that they have experienced behavior in violation of the National Speech & Debate Association Harassment and Discrimination Policy or relevant tenets of the Code of Honor or Coaches Code of Ethics may complete the following form to report the violation. After submitting the form, you will talk with the Belonging and Inclusion Advocate (in the tab room), who will collect information from involved parties and recommend a corrective course of action. 
NOTE: YOU MAY COMPLETE THIS FORM VERBALLY BY ASKING FOR THE BELONGING AND INCLUSION ADVOCATE IN THE TAB ROOM. 
Please note that the Belonging and Inclusion Advocate is a mandatory Reporter.
The Belonging and Inclusion Advocate will keep information as confidential as possible, but ethically and legally, Advocates are required to notify authorities if there is a report of behavior that presents clear and imminent danger to yourself or others; or if there is a case of known or suspected child abuse or neglect; or in the case of known or suspected sexual violence.
