Dear Parent or Guardian,
Peoria Heights Grade School is very excited to participate with the program providing weekend snack packs for participating students at our school. This program is a collaboration between Peoria Heights, Glen Oak Christian Church, Midwest Food Bank, Church Women United-Peoria, local neighborhood churches, and individuals. It is free of charge, voluntary, and requires no long term commitment from you or your child.
Snack Packs provide students with food items that can be eaten over the weekend. Each Snack Pack contains non-perishable items that are distributed every Friday or on the last day of the school week. The items will be randomly available based on donations and items purchased but could include food such as Ramen noodles, instant oatmeal, breakfast bars, hot chocolate, peanut butter, or cheese crackers, fruit cups, or pudding. Due to the storage process, we are unable to guarantee products have not come into contact with peanut butter during storage.
If you are interested in having your child participate in this voluntary program, please respond to the questions on this form. Families with siblings/multiple students at PGHS may include all students on the same form. Families may opt in or out of the program at any time by calling the grade school office at 309-686-8809. Snack Packs provided by the school are intended to be opened and consumed only at home. For the safety of all students and to maintain a clean environment, snack packs may not be opened on the school bus, shared with other students, or stored in lockers.
