In Town Adventures *

I, the parent/guardian, give ongoing permission to Shooting Stars Out of School & Daycare to take my child on the below field trips and am aware of the inherent dangers of participating in these events. The transportation used for in-town field trips will be walking to and from the designated field trip. Proper staff-to-child ratios will be maintained at all times to ensure the safety and supervision of all children.
