Pink in the Park – Breast Cancer Awareness Walk Liability Waiver and Release Form

1. Acknowledgment of Risks:
I, the undersigned, acknowledge that participation in the Pink in the Park Breast Cancer Awareness Walk involves inherent risks, including but not limited to slips, trips, falls, uneven terrain, exposure to weather conditions, insect bites, dehydration, and other hazards associated with outdoor activities. I understand that these risks may result in injury, illness, or other harm.

2. Assumption of Risk:
I voluntarily agree to participate in this event and assume full responsibility for any and all risks, whether known or unknown, arising from my participation.

3. Release and Hold Harmless:
In consideration of being permitted to participate, I hereby release, waive, discharge, and hold harmless The Way, The Truth, and The Life Christian Center, Inc., its officers, directors, employees, volunteers, and agents from any and all claims, demands, actions, liabilities, or causes of action arising out of or connected with my participation in this event, including any injury or damage caused by negligence.

4. Safety and Conduct:
I agree to follow all instructions provided by event organizers and to conduct myself responsibly. I understand that failure to comply may result in my removal from the event without refund.

5. Medical Treatment:
In the event of an emergency, I consent to receive medical treatment deemed necessary. I understand that I am solely responsible for any medical expenses incurred as a result of my participation.

6. Photography and Video:
I grant permission for photographs, video recordings, and other media of me taken during the event to be used by the church for promotional, educational, and informational purposes without compensation.

7. Binding Effect:
This release shall be binding upon me, my heirs, executors, administrators, and assigns.
