All families are required to acknowledge the terms outlined below; one form is required PER family.  The Family Authorization is an agreement between the school and all parents/guardians and students.

Handbook Agreement
It is essential that students and parents/guardians familiarize themselves with the policies in the St. Barnabas School Parent-Student Handbook. After reviewing the handbook completely, please verify your agreement to the policies within the document.

Technology Responsible Use Policy
Students of St. Barnabas School are expected to handle personal and school-owned technology with the utmost care. After reviewing the Technology Responsible Use Policy, please verify all students and parents/guardians in the household agree to the policies outlined throughout the document.

Universal Permission Form
We anticipate our teachers taking walks with the students, as needed, around the immediate area, from 103rd Street to 96th Street, Longwood to Seeley.  Walking trips will serve an educational or play-based purpose.  Students will be supervised by a St. Barnabas teacher or staff member at all times.

Family Volunteer Commitment
St. Barnabas is blessed to have many events that form a community that sets our school apart and brings joy to families. These events are only possible due to families sharing their time and talent. Families are expected to volunteer throughout the school year to ensure the continuation of events and activities. Opportunities are on our website. Your acknowledgment below indicates that you understand the expectation of volunteer participation.

School Directory Publication
To build and maintain and sense of community, St. Barnabas School will publish an annual online directory through
www.directoryspot.net. In a password-protected format, the school will share students’ name along with parent/guardian email and cell phone number.

Photo/Media Release
On occasion, St. Barnabas publishes photos/videos of our students on the school/parish website, social media page, print publications, in the local newspaper, and in other forms of communication. We do this to share information about our school and to keep parents and our community up-to-date on our many offerings and student accomplishments.

Asbestos Release
Please review the notification letter about Asbestos Content & Management Plan at St. Barnabas School. Should you wish to review the inspection report and management plan on file at our school, please contact Mr. Jonathan Stack at jstack@stbarnabasparish.org.

Opioid Antagonist Policy
Under Public Act 103-0348, which took effect January 1, 2024, public and private schools, including St. Barnabas School, are now required to keep a supply of an opioid antagonist on hand to be used in the event of an overdose. Please review this new policy HERE

Concussion screening
St. Barnabas follows the concussion protocol of the Illinois Elementary School Association (IESA).  Per our Athletic Board By-Laws, at the start of each school year, all students in Grades 3 -8 must participate in the free-of-charge concussion screening.  Screenings are conducted during the school day, and results are accessible to parents/guardians through the screening evaluators.
