Use of School Facilities Application


1.  Use this online form to make an initial request to use Moanalua Middle School facilities.

2.  Please be aware that ALL school events will take precedence (priority) over any request to use school facilities. Availability of the buildings/facilities is always subject to change. If a situation should arise that a request conflicts with a school event, your organization will be asked to cancel or reschedule an event.

3.  A signed Form UOP (Request & Agreement for Use of School Buildings, Facilities and Grounds) must be received by the school office at least TEN (10) working days prior to the requested date of use.

4.  This online application is NOT a substitute for a signed Form UOP, Moanalua Middle School will use this information for processing and planning.

5. Mail or drop-off the Form UOP to:
