If you chose the delivery option, please enter the FULL address of your delivery.  NOTE- you will be charged a $9.99 delivery fee  for Bloomfield, Nutley, and Belleville in NJ.  For deliveries outside of these Townships,  there will be an additional $1.00 fee per mile.  
Payment Policy: *
Payment must be made at the time you receive the invoice.  Venmo and Zelle are currently the only payment options.  
Order Policy: *
All orders are based upon availability.  An email will be sent to you in receipt of this form with further information.  I understand that all products used are based on availability and each board may vary in components.  Cancellations must be made 72 hours prior to delivery or pick-up date to receive a full refund.  Any cancellations made within 48 hours of delivery or pick-up date, will receive a 50% refund.  Any cancellations made within 24 hours of delivery or pick-up date will not be eligible for a refund.
