Certified Phlebotomy Technician Accelerated Hybrid Application

This enrollment form is for students applying to the 4-week Accelerated Phlebotomy Program, which combines online coursework with limited in-person requirements.

All lectures, homework, and quizzes are completed at home during the week from Sunday to Friday, we meet ONCE a week on Saturdays from 9:00 AM - 12:00 PM.

Please note: Your spot is not reserved until at least the $500 enrollment fee is paid (students can pay in full). Submitting this form alone does not guarantee placement in the program.

Following form submission you will see a list of links for the payment choice matching the choices you indicated on this application. Please be sure to complete your payment so that you may complete your registration.

Students who will be paying for their own tuition should use this form. Students who will be using HCAP, All Like - Hana Lima, or an approved third party should use the tuition assistance application form.

Class Duration Disclosure

While in-person class sessions are scheduled for up to three (3) hours, actual class time may vary depending on the number of students and the pace at which the day’s scheduled tasks are completed. On days with fewer students or quicker completion of required material, class may end earlier than scheduled.

Early release is not a reduction in course value. It allows students additional time to manage personal responsibilities and complete assigned coursework outside of class.

This does not constitute grounds for withdrawal or a refund—no exceptions.

National Exam Study Guide
Please note: The National Certification Exam is offered through the National Healthcareer Association (NHA) and is not included in your program tuition. However, I do offer a nonrefundable study guide to help you prepare for the exam, available for $50. 

If you pay in full you will be able to purchase the study guide with your payment today. If you elect to use a payment plan the cost of the study guide will be added to your weekly payment.

IMPORTANT PAYMENT POLICY:

Once your application is reviewed and approved, you will receive an invoice for the total amount (minus any applicable discounts on tuition only — not the enrollment fee).

You will have 24 hours to submit your payment. If payment is not received within that timeframe, your application will be discarded, and you will be required to reapply for the program.

If you PAY IN FULL no further payments will remain.

If you Pay $800 you will still be responsible for the other $800 in tuition, made in weekly installment payments.

If you Pay $500 you will still be responsible for the other $1100 in tuition, made in weekly installment payments.

Please note: Seats are only reserved for students who pay at least the NONREFUNDABLE enrollment fee within the 24-hour window. No exceptions will be made.

Payment Schedule.

PAYMENT SCHEDULE POLICY:
For students who do not pay in full at the time of enrollment, a payment arrangement is available. Students may divide their remaining balance into up to 4 payments, with the understanding that the full balance must be paid before taking the final exam—no exceptions.

The first payment is due the Friday before your first day of class.

All remaining payments will be due weekly on Fridays, by 11:59 PM HST.

A payment is considered late if not received by the due date and time.

Late payments will incur a $15 late fee.

Returned payments will incur a $25 return fee.

Students with unpaid invoices will not be permitted to attend class until the account is brought current. Failure to comply with this schedule may result in dismissal from the program.

By submitting this signed enrollment form and paying the enrollment fee, you are officially confirming your enrollment in Hawaii Medical Training Center’s Certified Phlebotomy Training Program.

Program Cost & Payment Breakdown

Total Program Cost: $1,600.00

Enrollment Fee: $500.00 (nonrefundable; includes registration and phlebotomy supplies)

Remaining Balance: $1,100.00

Refund & Withdrawal Policy

All withdrawals must be submitted in writing or by email. Text messages or verbal withdrawals will not be accepted.

If a student withdraws prior to the start of the first day of class, all monies paid will be refunded within 60 calendar days, minus:

The nonrefundable $500 registration fee, and

$300 per day for each day class was taught, regardless of student attendance.

If a student withdraws after the start of the first day of class, no refund will be issued—no exceptions.

Students dismissed or who drop out after the first day are still legally responsible for paying their remaining tuition balance in full.

Retake & Re-Enrollment Policy

Students who withdraw before completing the program may be eligible to re-enroll in a future course at a retake rate of $1,000, once their original balance is paid in full.

H.M.T.C. may apply the $500 registration fee toward a future enrollment if the student re-enrolls within 90 days of the original start date.

Collections & Financial Liability

Students with unpaid balances at the end of the program, and without an approved payment arrangement, will be referred to a collections agency.

This may negatively affect your credit score and future employment opportunities, as many employers conduct background and credit checks.

IMPORTANT: You are legally responsible for any outstanding balance, regardless of attendance or program completion. Communicate with the Director or Lead Instructor if you're having difficulties—we are here to help.

Class Cancellations & Schedule Changes

H.M.T.C. reserves the right to postpone or cancel classes prior to the start date due to low enrollment or other unforeseen circumstances.

If a class is postponed or rescheduled, we will make every effort to accommodate students. Schedule changes do not affect your enrollment status or financial obligations.
