2017 Brain Forest Student Application
Please use this form to enroll your student for the 2017 Brain Forest Summer Learning Academy. You must complete the whole application. **Your application is not submitted unless you have completed all pages of the application and see the message "Your application has been submitted."**

About the Program:
During the summer, children experience academic loss in reading and math. The goal of the Brain Forest Summer Learning Academy is to stop that loss. This academy is a 5 week academic-enrichment program for Montgomery Public Schools students. The program will be held from June 12 to July 21 from 8 am to 4 pm Monday-Friday. Please note, the program will not be held during the week of July 4th. Students may be dropped off as early as 7:30 and must be picked up NO LATER than 5 pm daily. Enrolled students are expected to attend each full day of the program. Students will be served breakfast, lunch, and a snack daily. Students will attend a weekly field trip every Friday.

The program will feature mornings of math and reading instruction with a certified teacher and a classroom assistant in each classroom. Instruction is centered around reading and math including a novel study with hands on, engaging learning activities. All students are tested at the beginning and end of the program to help teachers with instruction and to show student performance. Students will spend each afternoon in enrichment activities such as swimming, horseback riding, music education, kayaking, archery, financial literacy courses, sports activities, and more.

This program is provided COMPLETELY free of cost to all families. If you are enrolling more than one student, you must fill out this form separately for each student. All applicants will be notified by BY EMAIL by May 12th whether they are enrolled or waitlisted for the program. We will continue to accept applications until the first week of the program.


Program Application Requirements:
-Students must be enrolled in a Montgomery Public School
-Students must live in Montgomery
-Students must be GOING INTO the 4th, 5th, or 6th grade for the 2017-2018 school year.
-Parents must attend Parent Orientation on either June 7th or June 8th
-Parents must provide their child's most recent report card to the program at Parent Orientation
