Cedarbrook Fall Sports Interest Form (2025-2026)
This form must be completed for any student who would like to try-out for a fall sports team at Cedarbrook Middle School for the 2025-2026 fall sports season. The information entered into this form will be provided to the Cedarbrook co-Athletic Directors and Coaches.

Additionally, a completed PIAA Sports Physical (dated on or after May 1, 2025) must be turned in to the Cedarbrook co-Athletic Directors before any student may try-out for any team. Blank copy of the forms are available for download on the Cedarbrook Athletics website.
