Thank you for your interest in enrolling in the Grade A Plus, Academic, Enrichment, and Mentoring Program for high schoolers.
STEP ONE: APPLICATION
The application form provides space for information for one high school student. The information this form asks for is to help staff organize the information received and to provide data as required by our program funders. Please be sure to review each section before submission.
STEP TWO: LIABILITY WAIVER
After submission, a liability waiver will be emailed to the parent whose email is listed in the form. Parent is required to digitally sign the form before student can engage in the program.
