High School Student Program Enrollment Form
Thank you for your interest in enrolling in the Grade A Plus, Academic, Enrichment, and Mentoring Program for high schoolers. The activities we provide are MathUp! (9-12th grade math classes) and the Employment Opprotunity Program (EOP). 

STEP ONE: APPLICATION
The application form provides space to receive information for one high school applicant per form. The information will help staff organize the information received and to provide data as required to our program funders. Please be sure to review each section before submission.

STEP TWO: LIABILITY WAIVER
After submission, a liability waiver will be emailed to the parent whose email is listed in the form. Parents are  required to digitally sign the form before the student can become engaged in the program.

When the  Family Application form is submitted,  please send a message through the "CONTACT US" box on our website to alert us about this  application.  (https://www.gradeaplusinc.org/contact).  Thank you for your interest in this program. We hope to meet you and be able to provide service and support for your child(ren).
