The undersigned Parents/Guardians (the “Payor) of the above-named child agree to the following contract and to pay to Stoney Creek Children's Club ("the Club"), all fee payments required and are listed on the website.

There is no refund or reduction in fees in cases of withdrawal, dismissal, absence, cancellation, or persistent absenteeism. Fees are payable via our website, under the product section.

Registration is complete only upon receipt of a completed registration form and full payment for the desired weeks.

I/We acknowledge that the above information is complete and accurate to the best of my/our knowledge. I/We understand and acknowledge that providing false or incomplete information may have a detrimental effect on my child's participation in the Club, and may include an offer of admission being rescinded or dismissal from the Club. I/We also agree to provide to the Club, in a timely manner, any changes regarding my/our child’s information.

I/We, the undersigned, understand that the responsibility of the Club for the child begins when the child has been admitted to the program each day and ends when the child has been dismissed to go home.

To the best of our knowledge, our child is in good health (subject to information provided above). If we cannot be reached at a time of illness or accident, or if the emergency is such that time does not permit such contact, the Club is hereby authorized to contact the EMS to care for, hospitalize, and order injections, anaesthesia, or surgery for our child as needed.

We understand that the Club will notify us of excursions including walks to the local splash pad/community centre. We understand that beyond these two locations, the camp will occur in and around the premises of Battlefield Park including nature walks to the castle, museum, creek, and other parts of the premises.

We understand that young children, even under close supervision, have occasional accidents. Therefore, we hereby release, indemnify, and hold harmless the Club, its directors, agents, and employees from any and all claims, damages, or other liabilities for injuries to our child which are not a direct result of negligence of the Club, its directors, agents, or employees, or are entirely beyond the control of the Club, its directors, agents, or employees.

Stoney Creek Children's Club uses e-mail and other means of electronic telecommunication to distribute information about the Club. By signing this contract agreement, you are providing Stoney Creek Children's Club with consent to communicate with you via electronic means of telecommunications. If, at any time, you wish to revoke this consent, please state such intention in writing to the administration team.
