Baptism Application - St. Mary's Corvallis

Parents complete the entire form, sections 1-3 (provide the information for the child, parents, and Godparents whom you've selected).
Godparents complete the information for Godparents in section 3.

Note: this is for infant baptism. If you seeking baptism for an child older than 7 years of age, please contact the Religious Education director found under the Ministries tab.  If you are an adult seeking to enter the Catholic Church, fill out the application in the RCIA section.

Additional requirements
- Take the baptismal class (our parish staff will contact you after you fill out this application and provide the information about the class).
- Submit a copy of the child's birth certificate
- There is no fee for baptism. A free-will donation to the parish is suggested.

3. Godparents

Godparents have the mission of being role models for the child of what it means to live out the Catholic faith. Accordingly, godparents need to be confirmed Catholics who are practicing the faith. They need to be married in the Church if they live with a partner. Godparents are asked to take the baptismal class along with the parents.
