Directions:

Reports of bullying or harassment that impact the learning environment or compromise student safety are taken very seriously. Once this form is submitted, the Social Worker and/or the Compliance Coordinator at the student’s school will be notified. Every reported incident will be thoroughly investigated, and parents will be informed of the findings.

Anyone may submit a report. All school employees are required to report any alleged violations. If desired, this form can be submitted anonymously by leaving the "Name of Person Reporting the Incident" field blank.

Los informes de acoso o intimidación que afecten el entorno de aprendizaje o comprometan la seguridad de los estudiantes se toman muy en serio. Una vez que se envíe este formulario, se notificará al trabajador social o al coordinador de cumplimiento de la escuela del estudiante. Se investigará a fondo cada incidente denunciado y se informará a los padres sobre los resultados.

Cualquier persona puede presentar un informe. Todos los empleados de la escuela deben informar sobre cualquier presunta infracción. Si lo desea, puede enviar este formulario de forma anónima dejando en blanco el campo "Nombre de la persona que informa el incidente".
