Meal Request Form For In-Person Students Returning to School on December 9th: December 8th Pick Up
Grant Public Schools will be providing meals for students during the remote-only instruction period. This week's bag will include 2 breakfast meals and 2 lunch meals per student. Please complete a separate form for each of your GPS students. This form will close at NOON on Friday, December 4th. **Food allergy: A doctor's note and paperwork will need to be on file with the district before any component of a meal can be substituted.
Meals MUST be picked up on Tuesday, December 8th ONLY between the hour of 8:30 am - 11:30 am in the Middle School South parking lot/8th grade doors.
