I choose to participate as a volunteer for Hampton Roads Chess Association. I understand these services are being offered on a voluntary basis without anticipation of any financial reimbursement. I agree that I will abide by any rules and direction and will report any injury or incident immediately to HRCA staff.
I give my permission and consent to HRCA to use, print, copy, publish, and reproduce any and all videotapes, audiotapes, photographs, films, negatives, prints, reproductions, and likeness of any kind now or hereafter of myself made by HRCA for advertising, publicity, display without fees to be paid to myself and/or any child listed on this application. I hereby waive any right I may have to inspect and/or approve the finished product or the advertising copy that may be used in connection therewith or the use to which it may be applied.
By volunteering with HRCA and signing this Volunteer Application Form you are consenting to your information being collected and stored for the purposes of volunteer engagement. HRCA is committed to protecting the privacy of personal information of all stakeholders in the organization including volunteers. HRCA does not provide names and contact information of volunteers to other organizations or companies without prior explicit consent except to the extent required by law.
