Once you've reviewed the information and wish to proceed with testing, simply submit this form.* All of the information on the form is helpful for me to initiate the electronic application for your child. 

Each day I'll review this form for new submissions and send parents the applications via email.** The emails are not sent from my email address but through FISD automated system. Please check your spam or junk accounts for this email in order to complete the application.

All applications must be submitted on or before May 8, 2024 for testing Window A. No late applications will be accepted.  

*I do not have a syllabus, testing guide, or study material for this test.
**Please be mindful that I spend the school day hours counseling students. Therefore, testing requests are handled after school. Thank you for your patience.

Next Steps
  1. After this form is submitted, I'll initiate the application for your child. Per FISD guidelines, I will only initiate applications during the application window of April 15, 2024-May 8, 2024. You'll receive an email to complete the EA Application within 24 hours of submitting this form. 
  2. Please complete and submit the Exam for Acceleration application on or before May 8, 2024. 
  3.   4. Once the student has completed the test, scores will be sent to families by the district.
