Cancelation and Hold Request Form
**ALL CANCELATION REQUESTS REQUIRE a 30 day notice. Any invoices scheduled to charge within that 30 day window will be billed in full.  I understand that termination of my membership before the end of my contract will require a termination fee of the difference between the membership I chose and the membership I should have been on as outlined in my membership agreement. No refund of any payments made will be given.**

**ALL HOLDS can be requested in 1, 2, or 3 month increments. I understand that hold requests must be submitted no less than 5 business days before my upcoming scheduled invoice and I cannot place a hold on my membership for more than 3 months in a calendar year. I understand that my membership and payments will resume automatically upon expiration of the hold period I have selected, and the agreement duration will be extended for the amount of the hold time period requested.  I understand that if I cancel my membership during the hold period, the cancellation policy agreed to above applies.**

**Any request forms submitted after the required date will be applied to the next month and your upcoming bill will be processed as normal.**

I also understand that a staff member will reach out to me to follow up, and that my requested cancellation date is subject to our gym's policies and procedures. If you decide to reactivate your membership in the future (and we hope you do) membership rates in effect at the time of reactivation will be applicable as membership rates are subject to change. *
