By submitting this form you agree that you understand and acknowledge on your own behalf and/or certify that you have the authority to acknowledge on behalf of your organization, that you and/or your organization are solely responsible for implementing any safety protocols at your event(s) or gig(s) (referred to collectively as “event”) relating to the 2019 novel coronavirus disease (“COVID-19”) including, without limitation, protocols relation to vaccination, testing, facial coverings, physical distancing, sanitation, hygiene, and air quality and filtration methods. New England Conservatory (NEC) does not guarantee or certify the vaccination status of any musicians that participate in the Music Referral Service and does not verify whether or not any musician you elect to hire for your event has recently has recently been exposed to, or contracted, COVID-19, or is subject to isolation or quarantine expectations. By submitting this form, you, on your own behalf and on behalf of your organization, if applicable, agree to hold harmless NEC, and its officers, directors, employees, agents, successors, and assigns (collectively, "Releasees"), from any injury, illness, disability, and/or death that may arise should a musician referred for your event have been exposed to, or have contracted, COVID-19, whether caused by the ordinary negligence of NEC or any Releasee. You covenant not to make or bring any such claim against NEC or any other Releasee, and forever release and discharge NEC and all other Releasees from liability under such claims.
