Please complete this form to request a hearing screening / evaluation for your child through San Diego Unified School District's Audiology Assessment Center.  

It is required that your child have an up-to-date hearing assessment completed within the past 12 months in order to complete any assessments for special education (IEP).  If you do not have a record of recent hearing evaluation or your child has not passed a hearing evaluation in the last 12 months, your child will need another hearing evaluation in order to complete the special education evaluation process.

You may request a hearing evaluation through your pediatrician using your child’s medical insurance OR you may request a hearing evaluation (free of charge) at the Audiology Assessment Center at San Diego Unified School District by completing this referral form.
