物質名*1 / 形態（形状）*2 / 量および個数*3 / 危険性*4 / 拡散防止及び処理*5

※1  組成も記入すること。略称不可。薄膜の場合、膜組成、基板サイズも明記して下さい
　  例：　Sr0.9La0.1TiO3、1wt%Pt/Al2O3、ZnO（膜厚100nm）/SiO2（基板10*20mm厚さ1mm）など
※2  形態とは持ち込む時の状態、形状とは中の物質の状態をいう。
　  例：　ペレット（非晶質粉末）、アルミ製ホルダ（溶液）、プレート（多結晶薄膜）など
※3  1試料あたりの量及び個数。
       例：　100mg／10個、1g／5枚など
※4  例：　毒物、劇物、有機溶剤、特定化学物質、毒性、可燃性、無害など
※5  例：　ビニール袋に密封し、破損時の漏洩対策を講じて慎重に取り扱う

Information in this section is used for technical review; so please be as specific as possible.
*1: Enter the composition as well. Avoid abbreviations.
          e.g. Sr0.9La0.1TiO3, 1wt%Pt/Al2O3, 
                 ZnO (film thickness: 100 nm)/SiO2 (substrate size: 10 x 20 mm, thickness: 1 mm) etc.
In the case of thin films, also specify the film composition and substrate size.
*2: Specify the type of container and form of sample.
e.g. pellet (amorphous powder), aluminum bottle (solution), plate (polycrystalline thin film), etc.
*3: Use appropriate units. (e.g. 100 mg × 10 pieces, 1 g × 5 sheets, etc.)
*4: e.g. poison, deadly poison, organic solvent, specified chemical substances, toxic, burnable, harmless, etc.
*5: e.g. sealed in a plastic bag, handle with care and take measures against leakage in case of damage

例：
1.Pt/箔/1g/1個/無害/ビニール袋に密封した状態で取り扱う
2.PtO2/10mg/1個/無害/ビニール袋に密封し、破損時の漏洩対策を講じて慎重に取り扱う
3.1wt%Pt/Al2O3/粉末（ペレット）/100mg/10個/無害/ビニール袋に密封し、破損時の漏洩対策を講じて慎重に取り扱う
※必ず持込み試料についての<<測定試料の記載方法>>をご確認の上、ご記入ください。

e.g. 
1. Pt / foil / 1 g / 1 sample / harmless / sealed in a plastic bag
2. PtO2 /10 mg / 1 sample / harmless / sealed in a plastic bag, handle with care and take measures against leakage in case of damage
3. 1wt%Pt/Al2O3 / powder (pellet) /100 mg / 10  samples / harmless / sealed in a plastic bag, handle with care and take measures against leakage in case of damage

測定条件について Details of Measurement

※測定は室温、大気中で実施されます。

Please note that measurements are performed in air at room temperature.

試料毎の測定内容 Details of Measurement for each sample

試料ごとに測定元素、測定吸収端、XANESまたはEXAFSを記入してください。

例：1.  Pt箔, Pt, Pt-L3, EXAFS
　　2.  PtO2, Pt, Pt-L3, EXAFS
　　3.  1wt%Pt/Al2O3, Pt, Pt-L3, EXAFS
　　または、全ての試料, Pt, Pt-L3, EXAFS 

For each sample, specify the element, absorption edge, and XANES or EXAFS.

e.g.  1.  Pt-foil, Pt, Pt-L3, EXAFS
　　 2.  PtO2, Pt, Pt-L3, EXAFS
　　 3.  1wt%Pt/Al2O3, Pt, Pt-L3, EXAFS
