Ngāti Mākino Discretionary & Hauora Grant Application Form 2020
Purpose
Improve the health and wellbeing of Ngāti Mākino
Allocation
Individual grants will be paid at the trustee’s discretion
Eligibility & Criteria
* Applicant must be registered on the Ngāti Mākino Iwi Register or is a direct descendant of someone who is registered on the Ngāti Mākino Iwi Register

* A Ngāti Mākino Hauora/Discretionary Grant can be applied for from 6 January 2020 to 11 December 2020

* Contribution towards the cost of prescription glasses, dentures, smoking cessation and hearing aids

* Contribution towards the repair of prescription glasses, dentures, smoking cessation and hearing aids

* To support representation at the provincial or national levels, e.g. sporting or leisure pursuits

* The grant can be paid directly to the health provider or an applicant can be reimbursed upon providing a receipt from a health provider

* Only 1 grant is payable to an applicant in a 12 month period

* The Ngāti Mākino Heritage Trust may decline any application on any ground it sees fit

* Approval of any grant is entirely at the discretion of the trustees
Applicant
* Provide a whakapapa if required

* Must complete a Hauora/Discretionary Grant form

* For a Hauora grant, an applicant must attach a quote or receipt with their application

* For a discretionary grant, an applicant must provide the supporting information

* Write an essay about their future goals including how they will help Ngāti Mākino

* Must be willing and make time available to attend Ngāti Mākino wananga/hui
