1. What is the goal of your project? *
Explain the goals of  your proposal - what problem are you aiming to address? Be specific with regard to the communities which will be engaged, on what timeline, and with what tools.
2. What form will your project take? Examples include an app, an interactive webpage, or community located access opportunity *
3. What is the status of high-speed connectivity in your community? Please elaborate on the deployment of wired and wireless broadband access, as well as universality of coverage. *
Preference will be given to communities with ubiquitous broadband connectivity.
4. To what extent will your proposal integrate community input in identifying a problem and enunciating a solution? *
We have a strong preference for programming that will devolve meaningful control of community decision making to community members.
5. How will your community be partnering with local non-profits, private partners, or community anchor institutions? *
If so, please specify the organization(s); describe their mission, and explain the role they will fill
6. What is your plan to use high speed broadband connectivity to address the community issue you have identified? *
7. How will you quantify success if your proposal is funded? *
8. What qualitative measures of success will you be capturing, if funded? *

Communities applying will be expected to demonstrate how, in addition to the award funding, they will source external funds or use in-kind support. Please elaborate here.
