Team Central Waiver-2017
Welcome to Team Central. The information in this document explains your rights and obligations. Please make sure you understand it before signing and feel free to ask any questions you may have.
“Team Central” means the coaches, administrators and others working on behalf of Team Central. Team Central may also be referred to as “Us” or “TC” in this document.
“You” means the player, the player’s parents and/or legal guardian.
“Team Central season” means the series of practices, scrimmages and clinics organized by Team Central during a given calendar year.
Elite Teams means the teams organized by Team Central for participation in tournaments and other games.
Participation in the Team Central season/Elite Teams/Tourney Teams: You are signing up to tryout with Team Central for clinics, practices and scrimmages that will be available from December, 31 2015 through December, 31 2016 (the Team Central season). Purchasing or receiving Team Central apparel or equipment does not guarantee participation on any Elite Team. The Elite Teams are selected by ability, character, effort and commitment. Players must continue to exhibit good character, effort and/or commitment at all times (on or off the field and in the classroom) or risk dismissal from our program at anytime. Although we do our best to provide equal playing time when possible, we do not guarantee that players will get equal playing time in any TC event.
Eligibility – Team Central is an elite program for committed lacrosse players. We reserve the right to dismiss any player from the program at any time without refund. By participating in the Team Central season you agree that you will make every effort to be available if selected to participate on an Elite Team and that Team Central will be your primary lacrosse program outside your school. We do not encourage nor want players to play for other out of season lacrosse teams. We ask that you are committed 100% to Team Central. All tourney fees and forms must be returned by the date specified. Not returning fees and forms or being committed to Team Central could lead to elimination from events and/or the program with no refunds. There are no refunds for any reason.
Readiness to compete: It is your responsibility to determine whether you are physically and psychologically prepared to compete in a given event. You, not Team Central, are also responsible to ensure that you are wearing the appropriate protective equipment, (including mouthpiece, pads and footwear) appropriate for the playing surface and field conditions. You should inspect the inspect facilities and equipment prior to any TCLG event, practice, tournament, make coaches aware of any unsafe situations and, should not participate if you feel that they are unsafe. Your decision to participate means that you have determined that it is safe to play.
Medical Attention: You are responsible for making sure that you have any medications that you might require while participating in a TC event. You authorize Team Central, a host organization or sponsor of any event you attend as a member of Team Central to provide all medical/athletic training attention, transportation, and emergency medical services necessary to be administered for any accident, injury or sickness occurring during your participation in any TC event. TC, event sponsors or host organizations will take all reasonable efforts to manage your medical needs until your parent or guardian can assume responsibility for your care. You will be responsible for all costs arising from any such treatment or transportation.
College / School Planning: Team Central has no obligation to attempt to match you with a College or School program. However, by participating with Team Central, there is a possibility, but no guarantee, that a representative of Team Central may be contacted by a College or School representative regarding your suitability for their program. Typically, their questions include but may not be limited to, your athletic ability, character, and academic and standardized test achievement. By joining Team Central, you authorize TC to provide information based on our perceptions of your ability, character and academic achievement as we understand it. If requested by the College or School official, Team Central will ask you to provide transcripts, test scores and other official academic records to Team Central coaches and/or College/School representatives. It is your responsibility to obtain the requested information and meet any deadlines required by the College or School. Further, you understand that TC makes no guarantees or promises made relating to college or admission, financial aid, athletic aid, recruitment, playing time, graduation or even making the college/school team where you would like to attend.
Assumption of Risk/Waiver & Release of Liability: Lacrosse is a physical sport and Team Central cannot guarantee your safety while you are playing lacrosse. By signing below you acknowledge and agree that you are fully aware of and voluntarily assume the risks (including the risk of catastrophic injury and death) associated with participation in a lacrosse practice, scrimmage, clinic or game event.
In addition to this voluntary and knowing assumption of risk, you agree, on behalf of yourself, your parents, family, heirs and others, that Team Central, host organizations, and/or sponsors along with coaches, volunteers, employees, agents, officers and directors, and players of these and other event participating organizations and affiliates, shall not be liable for any injury, loss of life or any other damages or loss occurring as a result of your participation with Team Central. By signing this form, you give up all rights to pursue any action (legal or otherwise) for any reason, even if negligence was involved.
Binding Agreement – By signing below you and your parent or guardian acknowledge that you have read and understood every provision of this Agreement. You also agree to abide by these terms. If the participating player is a minor, my signature as parent or guardian indicates my understanding and acceptance of these terms on behalf of myself and my child. In addition, I will make a copy of this form for my records.
Refund policy: The deposit is NON-REFUNDABLE. If someone wants to request a refund for the 2nd or 3rd payments, they must do so in writing and it must occur before the start of the summer season: June 1st. The Directors of TC will consider serious injury, family emergency or re-location as possible reasons for refund, but will evaluate on a case by case basis.
Please note: Because we value the quality and character of the players who participate with Team Central, we reserve the right to dismiss any player from our program and/or events at anytime, without refund in the event their actions become a safety or behavior issue. In addition, we reserve the right to dismiss any player and parents at any time without refund if both or either are found or perceived to have questionable character, honesty and/or have any issues/participation/association with drugs, alcohol, felonies, misdemeanors, legal situations, violence, abuse, etc….whether it occurs during their participation with Team Central or at anytime, 365 days, 24 hours a day.
