The purpose of this survey is to gain your feedback about the topics of sessions that you would be interested in attending at this year's (or future) Spinal Cord Injury Forum, hosted by MetroHealth Medical System and the United Spinal Association Northeast Ohio Chapter.  The mission of our chapter is to provide educational and recreational opportunities to ALL individuals living with SCI/D.
