Following a successful interview, please complete the 'Promise of Employment' section of the Massachusetts Youth Work Permit (available at  https://www.mass.gov/info-details/youth-employment-permit-information). The student will then submit the form to authorized AB staff to receive a work permit number. Once processed, the original form will be returned to you for your records.
