Your Dogs Details
Before your dog/s can stay with us, we need to collect you and your dogs details, vaccination certificate (emailed or WhatsApp), microchip number, habits, behaviours, feeding time, amount of food, any medication, etc.
Please fill out one form per dog. This must be completed before your dog/s can stay with us stay, a lot of this information is a legal requirement.

What, how much and when?
It is a legal requirement for all dogs to to wear a collar with an ID tag? Even if the dog is wearing a harness with ID tag (The ID tag must have your surname name and full address, mobile numbers and the dogs name is optional
Is your dog good with children?
Is your dog good with other dogs?
Does your dogs have favourite thing: (food/balls/soft toys etc)?
Is your dog good off the lead, and has a good recall away from distraction? *
Are you happy for your dog to be let of the lead when walking? This will only be done where appropriate. *
Does your dog demonstrate any resource guarding issue, if YES, please explain  (food/bones/toys) *
Does your dog have any behaviour characteristic we should be aware of e.g. does not like cats, kids, cars, other dogs etc, if none, please put NO *
What word do you used to ask your dog to go to the toilet?
Key words. Are there any key words that are not the expected words, e.g. sit, down, come is expected, but "over here" meaning "come"
