Parents/guardians should complete this form if their student is going to be absent for a family vacation, college visit, special event or tournament -- anything that requires the student to be gone for an entire school day or more. Submit this form AT LEAST three school days prior to the intended absence.  For emergencies or unexpected events that prevent a three-day notice, please call the Attendance Line at 720-887-1992, option 1, and provide details regarding the absence.
