Inclusion in STEAM | Registration Form 2018
Please read this section in total before continuing to the questionaire:

This is a registration page for the upcoming workshop "Inclusion In STEM: Why Intersectional Feminism Matters in Science, Tech, Engineering, and Math." which will take place at the Democracy Center on October 27th from 10 am to 2 pm.

This form is step-one in the requirement for securing your enrollment in the workshop. Step-two is purchasing a ticket from Eventbrite (https://antiracismtraining.eventbrite.com) and/or contacting information@antiracismcollaborative.org if you have a sliding-scale need or to confirm your means of payment via cash or personal check.
(While we aim to be flexible we are not admitting folks to this event without completing this form on the date of the event as we must be able to confirm there is still a spot because most of our registrations are processed digitally).

The form results will remain confidential and are used solely to support your ease of access to the facilities and program content. We use these details to create the optimum program for your needs by sharing important information with the facilitators who will be working directly with you.

Please note that this form has been created in advance of our typical enrollment period due to demand and that you will hear from a course organizer closer to the date of the class for your participant resources. We fully support the movement to include Arts as a critical aspect of the STEAM field and areas needing improved inclusion and plan to add this to our time together.
